Japan's most famous fictional surgeon was created by a man who could have been a real one. Osamu Tezuka finished medical school in Osaka in 1951 and passed the national medical exam the following year, then spent his life drawing manga instead. In 1973 he started Black Jack, about a surgeon with no license, a scarred face and fees nobody can afford, who operates on the patients that licensed doctors turn away.

The habit has outlived him. Ameku M.D., the newest show here, comes from novels by Mikito Chinen, a practicing internist. Between those two, doctor anime have moved into the bloodstream, an imperial palace in an imagined China, and fantasy worlds that have never heard of germs. That is the order here: three shows that keep a patient on the table, two that go inside the body, three set in worlds before modern medicine, and two about diseases nobody can name.

Two of the ten are not doctors at all, strictly speaking. Maomao in The Apothecary Diaries is an apothecary and Farma in Parallel World Pharmacy is a pharmacist, but both spend their stories diagnosing, which is why they are here. Maomao's show is one of the two I have finished, with Monster; the other eight I describe from their premises and credits.

1. Black Jack

FormatGenreLengthEmotional tags
Anime seriesMedical, DramaTwo TV series (2004–2006)Moving, Tense

Black Jack is a genius surgeon with no medical license. He works out of a house on a cliff with Pinoko, a girl he assembled from organs that had grown inside her twin sister's body, and charges fees that only the desperate will pay. Most episodes are a single patient and a single operation.

Based on Osamu Tezuka's manga, which ran in Weekly Shōnen Champion from 1973 to 1983. This TV version was directed by Tezuka's son, Makoto Tezuka, with Satoshi Kuwahara. This is the first opening, Gekkōka by Janne Da Arc, from Tezuka Productions' official channel.

The Patient on the Table

The manga is more than 240 self-contained stories, each a single chapter, so the TV series works the same way: you can start almost anywhere, and each episode closes its own case.

From Japan: Black Jack is the obvious ancestor of the Japanese doctor who works outside the system. The brilliant surgeon the medical establishment will not license is an idea Tezuka made famous.

Japan in the late 1960s, during the student protests and the Vietnam War. Hazama Kuroo, a medical student with a scarred face and a talent no one at his university can match, takes on operations he has no right to perform yet. This is the story of how he becomes Black Jack.

A prequel manga written by Yoshiaki Tabata and drawn by Yūgo Ōkuma, serialized from 2011 to 2019. The anime aired in Japan from October to December 2015. This is the first promo, from TBS's official anime channel.

The Patient on the Table

Where Tezuka's Black Jack arrives fully formed, this one is set before the license question is settled, when Hazama is still inside a university and its rules.

The prequel manga ran in Young Champion, from Akita Shoten, the same publisher whose Weekly Shōnen Champion ran Tezuka's original.

3. Monster

FormatGenreLengthEmotional tags
Anime seriesThriller, Mystery74 episodesTense, Dark

Düsseldorf, 1986. Kenzo Tenma, a Japanese neurosurgeon on his way up at a German hospital, is ordered to operate on the mayor instead of a young boy who arrived first. He operates on the boy. Years later, people around Tenma start dying, and the boy he saved, Johan Liebert, is at the center of it.

Based on Naoki Urasawa's manga, which ran from 1994 to 2001. Madhouse adapted the whole story in 74 episodes, aired in Japan in 2004 and 2005. I could not find an official trailer that plays in every region, so there is no video for this one.

The Patient on the Table

I watched all 74 episodes, and the medicine never stops mattering. Tenma's belief that every life is worth the same is what puts him in danger, and the series keeps testing it until the last episode.

One warning before you start: it is a slow chase across Germany and the Czech Republic rather than a hospital drama, and most of the hospital scenes are in the first few episodes.

4. Cells at Work!

FormatGenreLengthEmotional tags
Anime seriesComedy, Educational21 episodes (2 seasons)Lighthearted, Fun

Inside a healthy human body, a red blood cell with no sense of direction tries to deliver oxygen on time, and a white blood cell keeps turning up to kill whatever got in. Each episode is one thing happening to the body: a scrape, a cold, an allergy, heatstroke.

Based on Akane Shimizu's manga, serialized from 2015 to 2021. David Production made a 13-episode first season in 2018 and an 8-episode second season in 2021. This is the official trailer from Aniplex USA.

The Body from the Inside

Here the doctor is the body itself. The cells are drawn as people, and the show names each cell type and what it does as it appears on screen.

Its success led to several spin-offs, and Code Black, the next entry, is one of them.

5. Cells at Work! Code Black

FormatGenreLengthEmotional tags
Anime seriesDrama, Educational13 episodesDark, Tense

The same idea as Cells at Work!, moved into the body of an adult man who smokes, drinks and does not sleep enough. A new red blood cell starts work in a body where the oxygen runs short, the bacteria keep winning and the staff are exhausted.

A spin-off manga written by Shigemitsu Harada and drawn by Issei Hatsuyoshiya, serialized from 2018 to 2021. Liden Films made the anime, which aired in Japan from January to March 2021. This is the first Japanese promo, from Aniplex's official channel.

The Body from the Inside

The title borrows the Japanese term for an exploitative company, a black company, and the body is run like one. The premise is the original show's cheerful workplace turned into a place that is breaking down.

It is a separate story with its own cast, so it does not need the original first.

6. The Apothecary Diaries

FormatGenreLengthEmotional tags
Anime seriesMystery, Historical48 episodes (2 seasons) + Season 3 airingClever, Absorbing

An imagined empire in the style of Tang-dynasty China. Maomao, who grew up as an apothecary in the red-light district, is kidnapped and sold into the imperial palace as a servant. When the emperor's infant children start falling ill, she works out why, and Jinshi, the eunuch who manages the rear palace, starts bringing her its mysteries.

Based on the light novels by Hyūganatsu. Toho Animation Studio and OLM made the anime: 24 episodes from 2023 to 2024, 24 more in 2025, and a third season that began on October 2, 2026. This is the main promo for Season 1, from TOHO animation's official channel.

A World Before Modern Medicine

I watched the first season, and what holds it together is that Maomao's answers are always physical: a poison, a food, a cosmetic, a plant. The palace intrigue is the setting, and her knowledge of medicine is what moves each case.

She is also the least respectful person in the palace toward anyone with rank, and much of the first season's comedy comes from that.

7. Doctor Elise: The Royal Lady with the Lamp

FormatGenreLengthEmotional tags
Anime seriesFantasy, Medical12 episodesHopeful, Romantic

Elise, an empress executed for her crimes, is reborn in modern times and becomes a surgeon. When that life ends too, she wakes up as her younger self in her original world, and sets out to become a doctor there instead of the woman who was executed.

Based on a South Korean web novel by Yuin and its webtoon adaptation drawn by Mini. Maho Film made the anime, which aired in Japan from January to March 2024. This is the main promo, from KADOKAWA's official anime channel.

A World Before Modern Medicine

The subtitle borrows Florence Nightingale's nickname, the Lady with the Lamp, so the English title alone tells you where her second chance leads.

It is the only show here based on a Korean original rather than a Japanese one.

8. Parallel World Pharmacy

FormatGenreLengthEmotional tags
Anime seriesFantasy, Medical12 episodesHopeful, Calm

A Japanese pharmacologist works himself to death and wakes up as Farma de Médicis, the ten-year-old son of a family of court apothecaries in a fantasy empire. Medicine there runs on magic and guesswork, and Farma opens a pharmacy to treat people with what he knew in his old life.

Based on the light novels by Liz Takayama, published since 2016. Diomedéa made the anime, which aired in Japan from July to September 2022. This is a promo from Animax Asia's official channel.

A World Before Modern Medicine

The premise is the reverse of most isekai: the hero's power is not a sword or a spell but a working knowledge of drugs, and the threats are diseases that his new world treats with magic or guesswork.

Like Maomao, Farma is a pharmacist rather than a doctor, and his new family are court apothecaries.

9. Dr. Ramune: Mysterious Disease Specialist

FormatGenreLengthEmotional tags
Anime seriesSupernatural, Medical12 episodesOffbeat, Warm

Ramune is a doctor who treats diseases that ordinary medicine does not recognize: illnesses that grow out of a patient's feelings and show up in the body in strange, visible ways. He travels with his young assistant Kuro and treats each one by finding what the patient is carrying.

Based on the manga by Aho Toro, which ran from 2017 to 2022. Platinum Vision made the anime, which aired in Japan from January to March 2021. This is the second Japanese promo, from King Amusement Creative's official channel.

A Disease Nobody Can Name

The diagnosis here is half medicine and half listening: the cure for each case depends on the worry or grudge that caused it, so every episode is also about the person behind the symptom.

The manga moved from a print magazine to Kodansha's Magazine Pocket app partway through its run.

10. Ameku M.D.: Doctor Detective

FormatGenreLengthEmotional tags
Anime seriesMystery, Medical12 episodesClever, Tense

Takao Ameku runs the department of diagnosis at a general hospital. She takes the patients other doctors have given up on, and the deaths nobody can explain, and some of those turn out to be murders.

Based on the ongoing novel series by Mikito Chinen, a practicing internist. Project No.9 made the anime, which aired in Japan from January to April 2025. This is the first promo, from Aniplex's official channel.

A Disease Nobody Can Name

The clues are meant to be medical ones: test results, symptoms and a patient's history, read by someone whose job is diagnosis.

The same novels were also made into a live-action TV drama in Japan in 2025.

Short Runs and Long Ones

Six of the ten fit into a single season. Young Black Jack, Parallel World Pharmacy, Dr. Ramune, Ameku M.D. and Doctor Elise are 12 episodes each, and Code Black is 13.

The long ones need a plan. Monster is 74 episodes on Netflix and does not let up, The Apothecary Diaries has 48 episodes across two finished seasons with a third airing now, and Black Jack's two TV series run from 2004 to 2006 on Prime Video.

Between the two I have finished, start with Monster's first episode. It is a single surgery and a single decision, and by the end of it you will know whether you want the other 73.

Streaming information was verified in the US region on October 21, 2026 and can change without notice.
